Meet Felix.

Felix was always able to carry a tune and enjoys listening to his old jazz records. Recently Felix's memory has started to fade and his family is worried about him being alone during the day and not treating his diabetes correctly. Our Adult Day Health Center can help Felix and his family by providing a range of activities to promote social, physical and emotional well being as well as provide quality monitoring of his daily health needs.

It's about changing how you live, not where you live.

Our Adult Day Health Services provide care and support for adults, (18 yrs and older) who, due to frailty or physical disability, require assistance during the day.

Daybreak provides a range of activities designed to promote social, physical, and emotional well-being.

Daybreak New
Adult Day Health Care is an organized program of services provided during the day for participants who live at home. But, it's much more than that - the purpose of Adult Day Health Care is to enable participants to stay at home for as long as possible, and to promote social, physical, and emotional well-being.

Our facility is located at
1407 Parkview Drive in Elizabeth City, NC.



Participants may be dropped off by family members, or transportation can be arranged. Our hours of operations are M-F, 7:30AM-5:30 PM.
